Information for New Patients
Make an Appointment
Please call (520) 742-7890 or (800) 637-4567 Monday - Friday 9:00am to 4:30 pm to make an appointment. Most new patients come to us by written referral or order from a physician or other medical professional. Please have your referral information available when calling for your appointment.
Prepare for your appointment
Please complete the new patient paperwork and bring it with your to your scheduled appointment. It is important to bring these forms, as well as your insurance card(s) and a valid photo ID.
If applicable, please bring the following items:
- Written order and/or Referral from your referring physician
- Medication list to include all prescription and non-prescription items (vitamin, herbal supplements)
- Radiology images and reports
- Medical records including office notes and tests performed
